How to use this log

Add one row per issue found during the pilot — from a teacher observation, student feedback, a parent question, or your own review. Log every issue regardless of severity; low-severity issues are useful evidence for the Outcomes Report even if no change is made.

Field guide

• Severity: Low (cosmetic/wording), Medium (confusing but workable), High (blocks a session or misleads students), Safeguarding (any real-disclosure or safety-adjacent issue — cross-reference the Safeguarding Protocol).
• Affected component: name the specific tool, mission or document (e.g. "Mission 5 — scholarship shortlist", "Diagnostic Q7").
• Decision: Accepted / Deferred to next pilot / Rejected with reason.
• Validation result: how you confirmed the fix worked — re-run with a student, teacher review, or "not yet validated".
